Health Quality Council - Thrive at Home Action Community

Our Health Service Committee also works closely with the Health Quality Council “Thrive at Home Initiative” whose goal is to strengthen and improve community-based care and services that enable older adults to “thrive at home” and age where they choose. The members include many Saskatchewan people representing the Saskatchewan Health Authority, provincial service agencies, service providers and seniors. Linked to resources from around the province, we meet regularly to share ideas, successes and challenges as we all strive towards similar goals for older populations.

Saskatchewan Health Quality Council Profiles Candle Lake's Work with the HQC Home Supports Action Hubs

"Home Support Action Hubs” are an initiative of HQC to find ways to "strengthen community-based care and enable Saskatchewan residents to age in place with dignity, independence, and a greater quality of life". Candle Lake Health Service committee members have been consistent participants.

Our Candle Lake Health Service Committee initiatives have been supported by HQC and in turn, we have been participating in the provincial working groups, the "Home Support Action Hubs" over the past year. 

We are committed to finding the "simple small town solutions" for the home supports that enable aging residents to "thrive at Home". We look forward to future joint initiatives as we work together with the province.


Sask Seniors Mechanism and Social Prescribing 

The Candle Lake Health Services Committee is participating in the Saskatchewan Social Prescribing Initiative. Saskatchewan Seniors Mechanism has selected Candle Lake along with 13 other Saskatchewan communities to receive funding for a 3-year project to demonstrate the value of this type of service and to introduce social prescribing to Saskatchewan.

Social prescribing is a holistic approach to health that links the healthcare system to community organizations that serve the needs of older adults (55+).

Amy Wingate, our Social Prescribing ‘Community Connector’ will work with the older adult community members with non-medical, health-related social needs (e.g. issues with housing, food, employment, income, social support, etc.). Her role is to help find non-clinical supports and services within the community and support individuals in accessing needed services. Amy can also help connect people to local services to improve their physical and mental well-being.

University of Saskatchewan College of Nursing and Kinesiology - Co-applicants with Candle Lake and recipients of the Align Grant

A $10,000 research grant to show that understanding of the local strengths and health needs, collaborative planning for collective actions and continuing re-evaluation of the  impact, local health services and supports can be augmented by the community itself. 

The Align Grant has provided Candle Lake with the consultative assistance of the researchers, and a student to assist in the processes and stages of the Candle Lake Alive and Well project

The results of this project will shine a light on the strengths, needs, and priorities of Candle Lake to support residents to thrive in their community. It will also provide structure to mobilize the knowledge created by community members, into action.

Additionally, the focus of this project will inform health and social sectors to support community-based care and service delivery closer to home for individuals living in rural areas and with complex care needs – especially individuals aged 55+ [66% of the population in Candle Lake]; a timely and important objective of the province.
