Social Prescribing - Community Connector
- Social prescribing is a means to connect patients to a range of non- clinical services in the community in order to improve their health and well-being.
- As opposed to simply treating symptoms, social prescribing may help to address underlying challenges to health and well-being. Social prescribing is a more holistic approach to health care, which promotes community-based integrated care and support.
Take advantage of this free service. Don’t struggle alone – access the support available through your Community Connector. She has successfully assisted numerous families in the community and can now accept referrals directly from individuals or family members with consent.

Red Cross Hospital to Home
- We understand that support for hospitalized patients on discharge is often required.
- Monitoring recently discharged people with a regular, scheduled phone call can prevent isolation, identify and find solutions for unexpected needs, and reconnect individuals and caregivers to community and health services when needs are identified.
- This Red Cross service is free, easily accessible and offered to anyone over the age of 18 years.
- Candle Lake Health Service is collaborating with the Red Cross, SHA leadership, our CL Health Center Staff, Physicians, the PA VIC Hospital to promote the service here and to the surrounding community. People can self-register or referrals can be made by SHA Caregivers, Physicians, Pharmacist, Medical First Responders, or our Community Connector.

Prince Albert Mental Health Centre: Outpatient Services & Walk-In Service
The Centre will provide same day access to a counsellor. Mental Health offers confidential community-based mental health assessment, treatment/counseling, and consultative services. Services can be accessed by family or self-referral, physician referral, agency or school referral. Psychiatric referrals are accepted by physicians only. Addiction Services - Rapid Access to Addiction Medicine (RAAM)
RAAM provides timely access to care for individuals looking to make changes in their substance use. At the RAAM clinic you will be seen by an inter-professional team who work together with you in creating a care plan that fits your needs and goals. RAAM services are short-term, and while you are with us, we will connect you to the appropriate community care providers for ongoing care and support.

Friendly Calls Program (Canadian Red Cross)
This Program matches people over the age of 18 with trained Red Cross personnel who connect with them regularly to provide emotional support, encourage healthy coping strategies, and suggest well-being resources and community connections to other existing services. The Friendly Calls program is safe, accessible and free. Referrals can be made by anyone. Click here to learn more.
Personal Disaster Assistance (Red Cross)
The Red Cross will assist to meet the immediate basic needs of people displaced by emergencies such as house fire or flood. This service involves providing short duration assistance (up to 72 hours) to an individual household or small number of households. Service delivery is facilitated by a Personal Disaster Assistance Team (PDAT) that is usually comprised of volunteer personnel. Call 1-888-800-6493
